Only an active Faculty/Staff member can submit equipment requests. Students and Retirees are not authorized to check out equipment.
All requests should be put in 48 hours in advance (M-F) and are subject to availability and staffing.

Equipment Loan Agreement

By signing/submitting this form, I agree to keep all loaned equipment in good condition. I agree to return the equipment promptly at the location and time listed above.

I am responsible for replacement costs if equipment is lost, damaged, destroyed or stolen. If I fail to return materials in good working condition by the return due date, the cost of repair or replacement will be due by department budget code or personal check payable to Information Technology. Failure to pay could result in deduction from my employee pay.

I acknowledge that I have read or heard this contract read aloud, and understand the conditions of the agreement.

Once submitted, we will review the request and notify you accordingly.
